1/31/2024 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, Jeremiah struggled to start the week off in a positive manner as he self reported he was involved in a physical altercation with his older sibling. Jeremiah reported he is tired of his sibling antagonizing him. Jeremiah reported he shoved his sibling 1xâs with moderate intensity during their 3 minute altercation. Behavior: Jeremiah then reported his sibling threatened him and that really angered him. He reported it was then that he shoved sibling 1x's with moderate intensity during the 3 minute incident before he felt bad for his actions and calmed down. Intervention: QMHP-C probed Jeremiah as to his request for todayâs individual counseling session. QMHP-C processed with Jeremiah how he needs to control his aggressive behaviors by not letting other dictate what he is going to say and do. QMHP-C reminded Jeremiah that when he feels himself becoming angry he should walk away from the situation and use the "Thought Stopping" technique. Intervention: QMHP-C processed with Jeremiah controlling his anger and using anger management techniques that were taught during some previous sessions will have helped him make the right decision during the incident today. QMHP-C also modeled and taught Jeremiah alternate ways to avoid conflict such as, talking things out as well as accept responsibility for his aggressive behaviors displayed them. Response: Jeremiah seemed receptive to todayâs session evidenced by his ability to verbalize understanding of session. Jeremiah stated the "Thought Stopping" technique only works when you have time to think but not when you are verbally attacked. Jeremiah stated he understands that everything has a process or steps that have to taken to eliminate a problem and he informed QMHP-C he will solve his problems appropriately. Jeremiah received verbal praise for behavior displayed during today's individual counseling session. Jeremiah utilized todayâs counseling session to identify other adaptive behavioral alternatives available to him to help him. Response: Jeremiah responded positively to being modeled and taught Jeremiah two ways to avoid conflict instead of acting out negatively. QMHP-C will continue to work with Jeremiah on decreasing all maladaptive behaviors at home and community. During the next scheduled counseling session, QMHP-C will establish a framework for treating others in a respectful manner and treat others with respect at least 4-5x's monthly.
